Annual Financial Reports Are Live
The Annual Authorization, formally known as the Annual Financial Report (AFR) is a yearly form that clubs and groups are required to complete. This process gives the group permission to use the 4-H Name and Emblem.
This form helps the state determine whether the club engages in any high-risk activities. Any shared information is used to update that clubs and groups page of the website.
This form is also to double-check clubs and groups’ financials. This process is a way for clubs and groups to do their taxes. Every group with a bank account has an EIN from the IRS. If the Annual Authorization is not returned, the group’s EIN can be turned off.

This form is due on September 1st to the Extension Office. Please include supporting documents as well. All are required.
- Wisconsin 4-H Clubs and Groups Annual Authorization (this document).
- Copy of the bank statement(s) ending in June and July for ALL accounts. The bank statement is collected to ensure funds are deposited appropriately.
- Current inventory of land, buildings, property, or project equipment with individual values of $2,000 or more.
