There is always a lot going on throughout the year with the 4-H program, and hopefully, this page will be a one-stop shop for all the forms and applications you need. Behavioral Concerns Form
This form is to be used to document behavior concerns of 4-H members or 4-H volunteers participating in 4-H programs, projects, or activities. IMPORTANT: this form is not to be used for Accidents or Injuries and is NOT a substitute for Mandated Report Procedures
Charter Farms
4-H Charters are very important for a 4-H Club, or chartered project, to have completed. This process should not be taken lightly and should involve all members of the 4-H club. It is a great opportunity to go over your club’s goals and to work as a team. Below is some helpful information for your club in completing the charter process. Donation Forms
If a 4-H Club or Group is chartered, that means that paperwork is filed by the 4-H Program Coordinator so that they remain a 501(c)3 non-profit organization. Being a 501(c)3, this means that donations given to chartered 4-H clubs or groups are tax-deductible. Below you will find a sample donation record and receipt form, and a sample tax donation letter receipt that you should utilize if your chartered 4-H club or group receives a donation.

Funding Request Forms
These forms should be used when an individual or group is requesting monetary support from the Trempealeau County 4-H program for 4-H-sponsored events. To ensure adequate review, please note the following important guidelines:
- Submission Deadline: All funding requests must be submitted before the date of the event for which funds are being requested.
- Meeting Schedule: Requests will be reviewed during the quarterly meetings, which are typically held on the 4th Monday of March, June, September, and December.
- Meeting Submission Deadline: To be considered at a quarterly meeting, the funding request must be received before the meeting date. Please plan accordingly to allow sufficient time for submission.
- Youth Participation: Youth members who are part of the funding request are encouraged to attend the meeting in person or virtually to answer any questions the committee may have.
- Post-Event Reporting: Following the event, individuals or groups who received funding are asked to submit a brief report to the committee detailing their participation and the outcomes of the funded activity.
After the Committee discusses the request, the requester may be asked to leave the meeting and will be notified by email of the Committee’s decision.
Livestock Educational Events Forms
For various projects, there are extra requirements that the youth involved in the project need to participate in. One of those extra requirements is YQCA, and the other is often proof of attendance at extra educational events. Use the form below for your proof of attendance at these extra educational events. These are due on July 1 to TrempCAC@outlook.com
